目的 研究甲状腺转录因子-1(thyroid transcription factor-1, TTF-1)和广谱角蛋白(pan-cytokeratin, CK)在肺小细胞癌(small eelllung carcinoma, SCLC)中的表达, 初步探讨其作为SCLC预后判断分子标志物的可能性。 方法 收集天津医科大学附属肿瘤医院2006年1月至2008年12月间, 经手术切除、病理证实的SCLC 65例, 采集病例相关临床病理资料。免疫组化EnVision二步法分别检测TTF-1及CK在SCLC中的表达, 并对结果进行分析。 结果 TTF-1和CK在SCLC中阳性表达率分别为61.5%(40/65)和69.2%(45/65)。Kaplan-Meier分析显示, TTF-1表达阳性者预后优于表达阴性者(Log-rank, P=0.012)。CK不同表达两组间生存差异无统计学意义。多因素分析显示, TTF-1表达、淋巴结转移、pTNM分期为影响SCLC患者术后生存的预后因素。 结论 TTF-1与SCLC预后密切相关, 可能成为SCLC临床预后良好的标志物。CK在判断SCLC预后中的地位尚需进一步探讨。 Abstract:Objective The aims of the current study are to investigate the expression of thyroid transcription factor-1(TTF-1) and pan-cytokeratin(CK) in small cell lung carcinoma(SCLC), and to explore the possibility of TTF-1 and CK as prognostic markers of SCLC. Methods The clinicopathologic data of 65 SCLC patients who underwent total resection in Tianjin Medical University Cancer Hospital between January 2006 and December 2008 were reviewed.The expression of TTF-1 and CK in SCLC tissue specimens were detected using immunohistochemical En Vision indirect method.The relationship between the expression of TTF-1 and CK and the clinico-pathological data was analyzed. Results The positive rates of TTF-1 and CK in SCLC tissues were 61.5%(40/65) and 69.2%(45/65), respectively.Kaplan-Meier analysis showed that the expression of TTF-1 was significantly correlated with the overall survival of patients with SCLC(log-rank test, P=0.012).Cox multivariate analysis exhibited that TTF-1 expression, nodal metastasis, and tumor node metastasis stage were prognostic factors. Conclusion The expression of TTF-1 may be positively correlated with the prognosis of SCLC patients.Further studies are needed to evaluate the prognostic value of CK in SCLC. -
