Abstract:
Objective : To investigate the risk factors and prognosis of ovarian metastasis from endometrial carcinoma.
Methods : Data from 764 patients with endometrial carcinoma who were treated in our hospital from September 1960 to August 2001 were retrospectively analyzed. All of these cases had complete records of surgery and pathology.
Results : Among these 764 cases, 23 (3.01%) cases had ovarian metastasis. Univariate analysis found that depth of myometrial invasion, ascites cytology, histological grade, cervical migration, parauterine metastasis, fallopian tube metastasis and lymph node metastasis were significantly different between the cases with ovarian metastasis and those without ovarian metastasis. Predictive factors of ovarian metastasis based on forward logistic regression analysis included parauterine migration, ascites cytology and the depth of myometrial invasion. Patients with ovarian metastasis had a poorer five-year survival rate and a higher recurrence rate than those patients without ovarian metastasis.
Conclusion : Parauterine metastasis, ascites cytology and depth of myometrial invasion are independent risk factors for ovarian metastasis of endometrial carcinoma. The prognosis of endometrial carcinoma with ovarian metastasis is poorer than that for endometrial carcinoma without ovarian metastasis.
