Clinical Significance of Survivin in the Carcinogenesis of Oral Submucosal Fibrosis

  • 摘要: 目的 :检测凋亡抑制蛋白Survivin在正常口腔粘膜、口腔粘膜下纤维性变(OSF)及OSF癌变组织中的表达,探讨Survivin的表达在OSF癌变过程中的临床意义及其与临床特征的关系。 方法 :收集10例正常口腔粘膜、40例OSF(早期10例,中、晚期各15例)及42例OSF癌变组织标本。采用逆转录聚合酶链式反应(RT-PCR)检测正常口腔粘膜、OSF及其癌变组织中Survivin mRNA的阳性表达率,并用免疫印迹Western blot分析Survivin蛋白在正常口腔粘膜、OSF及其癌变组织中的表达。 结果 :正常口腔粘膜上皮中无Survivin mRNA阳性表达;OSF组Survivin mRNA表达阳性率为47.5%(19/40),其中早期阳性率为30.0%(3/10),中期阳性率为46.7%(7/15),晚期阳性率为60.0%(9/15);42例OSF癌变组织中40例有阳性表达,阳性率高达95.2%。Survivin mRNA在OSF癌变组中阳性表达率显著高于OSF组(P<0.01);而OSF早、中、晚期三组间Survivin mRNA阳性表达率虽逐渐增高,但并无显著性差异(P>0.05)。Western blot结果表明,正常口腔粘膜上皮中无Survivin蛋白表达,OSF早、中、晚三期上皮组织中Survivin蛋白表达呈逐渐增强趋势,在OSF癌变组织中Survivin蛋白表达最强。这与Survivin mRNA的表达基本一致。SurvivinmRNA的表达与OSF癌变患者的年龄、肿瘤大小、肿瘤部位及临床TNM分期等均无明显相关(P>0.05)。 结论 :Sur-vivin在OSF癌变过程中具有重要的促进作用,有望成为OSF癌变诊断和治疗的新靶点。

     

    Abstract: Objective : To investigate the expression of Survivin protein, an inhibitor of apoptosis protein (IAP) family member, in normal oral mucosa, oral submucosal fibrosis (OSF) and oral squamous cell carcinoma (OSCC) o-riginating from OSF.We will also explore the clinical significance of Survivin in the carcinogenesis of OSF and the relationship between Survivin expression and clinical features of OSCC originating from OSF.Meth-ods: We used reverse transcription polymerase chain reaction (RT-PCR) and Western blot to detect the Sur-vivin mRNA and protein expression in tissues from 10 normal oral mucosal tissues, 40 cases of OSF (10 cas-es of early stage, 15 cases of moderately advanced stage, and 15 cases of advanced stage) and 42 cases of OSCC originating from OSF. Results : Survivin mRNA was not detected in normal oral mucosal epithelium.The rate of Survivin mRNA detection in OSCC originating from OSF (95.2%, 40/42) was significantly higher than that in OSF (47.5%, 19/40) ( P <0.01).No statistically significant difference was found in the rates of Survivin mRNA expression among the different stages ( P >0.05).Western blot analysis showed gradually increasing ex-pression of Survivin protein as the stage of OSF increased, which was consistent with Survivin mRNA expres-sion.For patients with OSCC originating from OSF, Survivin mRNA expression was not correlated to age, tu-mor size, tumor location or TNM stage. Conclusion : Survivin may play an important role in the carcinogenesis of OSF and may provide an index for diagnosis as well as a target for treatment.
