胸腔镜在肺良性肿瘤及瘤样病变诊断和治疗中的应用

Application of Video-Assisted Thoracic Surgery in the Diagnosis and Treatment of Benign Lung Tumors and Tumoral Lesions

  • 摘要: 目的:探讨胸腔镜(VATS)在肺良性肿瘤及瘤样病变诊断和治疗中的应用价值。方法:1992年10月~2002年10月共收治肺良性肿瘤及肺类瘤疾病患者103例,107次(双侧4次)VATS手术(包括小切口辅助18例和中转开胸4例)切除瘤体114个。其中男性62例,女性41例,年龄13~71岁,平均41.9岁;病变在右肺51例,左肺48例,双肺4例;VATS完成手术81例,小切口辅助下完成手术18例,中转开胸4例。手术方式:肺楔形切除73例77次(其中4例双肺病变行双侧VATS),肺叶切除21例,肿物切除8例,全肺切除1例。结果:本组无手术死亡病例,平均术后住院4.8天。术后病理:结核球31例(36个),炎性假瘤20例(22个),错构瘤16例(17个),慢性肉芽肿9例(12个),肺囊肿10例,硬化性血管瘤9例,曲霉菌球3例,脂肪瘤2例,软骨瘤2例,神经鞘瘤1例。结论:对肺良性肿瘤及肺瘤样病变,VATS是一种较好的诊断和治疗手段,可达到安全、微创、痛苦轻、恢复快的效果,将有望取代传统的开胸技术。

     

    Abstract: Objective: To determine the current role of Video-Assisted Thoracic Surgery (VATS) in the diagnosis and treatment of benign lung tumors and tumoral lesions. Methods: 107 thoracoscopic procedures were performed in 103 patients between Oct. 1992 and Oct. 2002. Among them 62 were males and 41 females, with mean age of 41.9 years (range 13-71 years). These consisted of left VATS in forty-eight patients, right VATS in fifty-one and bilateral VATS in 4 patients. A total of 114 tubers were resected. Results: All patients got definitive pathologic diagnosis and all tubers were resected with VATS, Eight of them were assisted with minimal thoracotomy and 4 were converted to thoracotomy. All patients recovered uneventfully. The average postoperative hospital stay was 4.8 days. There were no incidence of postoperative complication and mortality. Conclusion: The result of this study suggests that VATS is an acceptable approach in the diagnosis and Treatment of benign lung tumors and tumoral lesions.
