再谈肺癌中血管生成与其预后的关系

A Study on the Relationship Between Angiogenesis and Prognosis in Patients with Lung Cancer

  • 摘要: 目的:研究肺癌中血管内皮生长因子(vascularendothelialgrowthfactorVEGF)、激酶插入结构域受体(ki鄄naseinsertdomainingreceptorKDR)表达水平和微血管密度(Micro-vasculardensityMVD)及其与肺癌预后的关系.方法:采用免疫组化方法(LSAB法),检测114例肺癌组织中的VEGF、KDR表达水平及MVD.结果:肺癌组织中VEGF、KDR高表达组患者5年生存率分别为18.18%和21.19%,显著低于低表达组61.19%和52.74%(P<0.01);MVD高密度组患者5年生存率为17.27%,显著低于低密度组52.74%(P<0.01).VEGF、KDR表达水平及MVD与生存期均呈显著负相关(P<0.01),其相关强度依MVD、VEGF、KDR顺序逐渐降低.多因素分析表明,pTNM分期仍是预测肺癌预后最有价值的指标,但VEGF、KDR表达水平,MVD也是判断肺癌预后较有价值的指标.VEGF、KDR表达水平越高,MVD越大,其无瘤生存期和5年生存率越短.结论:肺癌中VEGF、KDR基因的表达异常及MVD的增大在肺癌的发生、发展、转移和预后中可能起重要作用,检测肺癌中VEGF、KDR基因表达水平及MVD有助于预测肺癌预后.

     

    Abstract: Objective: To study the relationship between YEGF, KDR gene and MYD and prognosis in patients with lung cancer. Methods: The expression of VEGF、KDR gene and MYD were detected in 114 lung cancer tissues by immunohistochemical staining (LSAB Methods). Results: The 5 -year surrvial rate with high expression of YEGF and KDR (18.18% 21.19%) in patients was significantly lower than those with Low YEGF, KDR expression (61.19%, 52.74%)(P<0.01); The 5-year surrvial rate with high MYD group (17.27%) in patients was significantly lower than those with Low MYD group (52.74%)(P<0.01); The survival of patients was negatively related to the expression of YEGF, KDR and MYD (P<0.01); The results of multivariable cox model showed that the pTNM was the most significatly variables for predicting prognosis among the all related factors in lung cancer, But MYD, KDR and YEGF are the more significantly variables. Conclusion: Overexpression of YEGF KDR gene and high MYD may play an important role in the oncogenesis, development of lung cancer; Detection of MYD and expressional of YEGF and KDR might be helpful to predict prognosis in patients with lung cancer.
