3000例中枢神经系统病变冰冻诊断分析

Diagnosis and Analysis on Frozen section of 3000 CNS Lesions

  • 摘要: 目的 :重新研究3000例神经系统占位性病变的冰冻切片;分析误诊病例之病理特点;归纳以脑肿瘤为主的中枢神经系统疾病冰冻诊断的要点、难点。 方法 :分析经术后石蜡切片诊断证实的3000例术中冰冻切片诊断结果。 结果 :冰冻诊断准确率95.87%(2876/3000),误诊率4.13%。误诊率最高的肿瘤是脑膜间叶性肿瘤、血管母细胞瘤、淋巴瘤、少突胶质细胞瘤等。 结论 :正确的冰冻诊断需要全面的临床资料和足够的病变组织;病理医师要有雄厚的病理基础和冰冻切片诊断的丰富经验及较全面的临床、影像学知识才能保证诊断的可靠性。

     

    Abstract: Objective : Reinvestigating frozen sections of 3000 cases of CNS disease; Analyzing pathologic characters of misdiagnosis case. Concluding the important and difficult points of frozen sec-tion diagnosis on CNS disease. Methods : The frozen sections of 3000 cases CNS disease are analyzed by comparing the frozen section with paraffin section and clinical data. Results : The accurate rate of frozen section diagnosis is 95.87%, the misdiagnosis rate is 4.13% in this material. The highest misdi-agnosis rate tumors are meningeal mesoderm sarcoma、haemangioblastoma, lymphoma、oligoden-droglioma, et al. Conclusions : The conect frozen section diagnosis needs to have enough samples and clinical data; Pathologic doctor must learn more basic pathological knowledge and rich experience on frozen section and clinic 、 radiographic knowledge, in order to ensure correct diagnosis.
