Abstract:
Objective : To analyze retrospectively the clinical factors affecting the survival of children patients with germinomas.
Methods : A total of 40 patients with germinoma had been treated between the period from January 1989 to December 2002. Calculation of the survival rate and the statistics analysis were respectively carried out for different nosogenic sites, Brodeur staging and AFP level of the serum, as well as the regimen of chemotherapy.
Results : In the group the median survival rate was three year and two months and the two-year event-free survival rate of malignant germinoma was 55.00%. The various primary foci seemed to have no effect on the survivals, while the survival rate of immature teratomas was higher than that of endodermal sinus tumors; the survival rate of stage I and II was higher compared to that of stage III and IV; the two-year event -free survivals of the cases with normal AFP level was higher compared to the cases with elevated AFP level. Chemotherapy with BEP/PVB regimen enhanced the two-year event-free survival rate, compared to other regiments.
Conclusion : With the progress of modern chemotherapy, the survival rate of the children extracranial germinoma, especially of malignant extracranial germinoma has been greatly improved and the clinical staging,initial AFP level of the serum before treatment and choice of chemotherapy regiments are important prognostic factors to affect the prognosis of children germinomas.
