The Relationship between Extrahepatic Bile duct Carcinoma and CD81,A Cell Receptor for HCV

  • 摘要: 目的:探讨肝外胆管癌与丙型肝炎病毒受体CD81的相关性。方法:采用逆转录聚合酶链反应(RT-PCR)和链霉菌抗生物素蛋白-过氧化酶法(SP法)检测62例肝外胆管癌及30例胆道良性病变组织中HCVRNA、CD81mRNA序列片段以及HCVNS5抗原、CD81蛋白的表达。结果:肝外胆管癌组织中HCVRNA和CD81mRNA的阳性率分别为58.1%和72.6%,与对照组相比差异有显著性(χ2=22.030,P<0.001;χ2=7.453,P<0.01);HCVRNA阳性的肝外胆管癌组织CD81mRNA表达率为55.6%,HCVRNA阴性的CD81mRNA表达率为96.2%,两者差异有显著性(χ2=12.503,P<0.001);免疫组化检测均有类似结果。HCVRNA阳性肝外胆管癌的CD81mRNA丢失与否与分化状况有关(χ2=14.103,P<0.01);分化Ⅳ级与Ⅰ级的CD81mRNA丢失差异有显著性(P<0.05/3)。结论:肝外胆管癌组织中有HCV基因的高表达;CD81作为HCV感染胆管细胞的受体与肝外胆管癌分化有一定的相关性。

     

    Abstract: Objective: To study the relationship between the extrahepatic bile duct carcinoma and the CD81, a cell receptor for hepatitis C virus. Methods: Nested reverse transcriptase-polymerase chain reaction (nested RT-PCR) and immunohistochemistry assays were used to detect the expression of HCV RNA, CD81 mRNA, NS5 and CD81 protein in tissues of 62 patients with extrahepatic bile duct carcinoma. The control group included 30 patients with benign lesion of hepatobiliary ducts. Results:The positive expression rate of HCV RNA and CD81 mRNA in tissues of extrahepatic bile duct carcinoma was 58.1% and 6.7% respectively. Compared to the group of benign hepatobiliary lesions, there was a statistically significant difference(P<0.01). The expression rate of CD81 mRNA in tissues of extrahepatic bile duct carcinoma with HCV RNA(+) and HCV RNA(-) was 55.6% and 96.2%, respectively and there was a statistical difference between the two(P<0.001). The results of immunohistochemitry assays were similar. The loss of CD81 in 36 cases of extrahepatic bile duct carcinoma with HCV RNA(+) was prominent and associated with the differentiation of extrahepatic bile duct carcinoma (X2=14.103, P<0.01). The difference of CD81 loss between high differentiated cases (Ⅰgrade) and poorly differentiated cases(Ⅲ grade)was significant(P<0.05/3). Conclusion: High expression of HCV gene can be seen in tissues of extrahepatic bile duct carcinoma. Loss of CD81 may be related to the differentiation of extrahepatic bile duct carcinoma.
