56例瘤周脑水肿与脑膜瘤增殖及术后复发的临床意义

Clinical Significance for the Correlation between Peritumor Brain Edema and Proliferation and Postoperative Recurrence of Meningiomas: A Report of 56 Cases

  • 摘要: 目的:探讨瘤周脑水肿(Peritumorbrainedema,PTBE)程度与脑膜瘤细胞增殖及术后复发之间的关系。方法:分析56例SimpsonⅠ～Ⅱ级切除的脑膜瘤患者,按术前瘤周脑水肿程度分为无、轻和重度水肿组,术后测定各组肿瘤增殖细胞核抗原(Proliferatingcellnuclearantigen,PCNA)表达水平,分析瘤周水肿程度、PCNA标记指数(PCNALI)及术后复发之间的关系。结果:56例患者中5例复发,复发率为8.93%,其中轻度水肿1例,重度水肿4例。36例可在术前CT或MRI观察到瘤周脑水肿,3组的PCNALI分别为(0.99±0.16)%,(2.87±0.36)%,(3.89±0.35)%,统计分析表明水肿程度与PCNALI及复发率之间有较好的相关性。结论:瘤周脑水肿程度与脑膜瘤的增殖和术后复发密切相关,是预测肿瘤复发的一个良好指标。

     

    Abstract: Objective: To study the correlation between the peritumor brain edema (PTBE) and the proliferation and recurrence of meningiomas. Methods: Fifty-six patients who underwent resection of meningiomas were followed-up during the period from Jan.1995 to Jan 2000. They were divided into three groups (naught, mild and severe PTBE) according to CT or MRI. Proliferating cell nuclear antigen (PCNA) was detected with immunohistochemistry. Results: Recurrence occurred in five cases (8.93%). Thirty-six patients suffered PTBE before operation and the PCNA label index (LI) and recurence rate of the severe group was higher than that of the other groups. PTBE was related to meningioma cell proliferation and recurrence after surgery. Conclusion: PTBE and PCNA LI can predict the recurrence of meningiomas after surgery.
