卵巢透明细胞癌伴高雄激素血症1例并文献复习

The Clear Cell Carcinoma of Ovary with Hyperandrogenism: A Case Report and Review of the Literature

  • 摘要: 目的:提高对卵巢透明细胞癌伴高雄激素血症的认识,避免误诊。方法:卵巢肿物伴高雄激素血症患者1例,临床表现闭经,男性化。结合文献对其临床、病理诊断及鉴别诊断进行复习。对卵巢非功能性肿瘤产生雄激素的机制进行探讨。结果:患者病理诊断为卵巢透明细胞癌。术后两周高雄激素血症恢复正常,对化疗不敏感,术后6个月死亡。结论:卵巢透明细胞癌伴高雄激素血症罕见,临床和病理应与分泌雄激素的卵巢间质细胞肿瘤、脂质细胞瘤及胃肠道转移瘤等其它恶性肿瘤和疾病进行鉴别。诊断主要根据组织形态学,免疫组化在鉴别诊断中有一定作用。

     

    Abstract: Objective: To heighten the apprehension of the clear cell carcinoma of ovary with hyperandrogenism and to avoid misdiagnosis. Methods: A rare case of primary clear cell carcinoma of ovary with amenorrhea and virilization was reported. Through reviewing the literature and discussing the mechanisms of hormone production by these tumors we want to knowhowto make differential diagnosis with other tumors. Results: A primary clear cell carcinoma of ovary was revealed by Histology. The abnormal hormone value was normalized two weeks after operation. But the case show an aggressive course and did not respond to chemotherapy. The patient died of the disease 6 months after the operation. Conclusion: Ovarian neoplasm, other than sex cord stromal tumor, is the rare cause of hyperandrogenism. Only three cases of primary clear cell carcinoma of ovary is associated with virilization have been reported. Differential diagnosis should be done with sex cord stromal derivation tumor, steroid cell tumor, yolk sac tumor and metastatic tumor. The main basis for diagnosis is histology. Immunohistochemistry may be helpful in differential diagnosis.
