  • 摘要: 目的:以MUC1和CK7为基因标志物检测乳腺癌患者外周血中播散的肿瘤细胞,探讨肿瘤基因标志物在乳腺癌微转移诊断中的意义。方法:采用RT-PCR法检测乳腺癌患者外周血中有核细胞的MUC1和CK7mRNA表达。结果:1)检测15例正常人外周血MUC1、CK7mRNA表达,结果无假阳性;将乳腺癌细胞系MCF-7分别以1:105、1:106与正常外周血有核细胞混合,检测MUC1、CK7表达,结果无假阴性。2)检测35例乳腺癌患者外周血,以MUC1为标志物,外周血中肿瘤细胞检出率为30.3%(11/35);以CK7为标志物,外周血中肿瘤细胞检出率为35.0%(12/35)。3)35例患者中有4例MUC1、CK7均表达阳性,7例仅MUC1表达阳性,8例仅CK7表达阳性,两种肿瘤标志物表达患者有交叉但不重叠。结论:以MUC1、CK7为基因标志物,采用RT-PCR方法检测乳腺癌患者外周血中的肿瘤细胞可能在乳腺癌微转移诊断及乳腺癌预后中具有意义,多种肿瘤基因标志物联合检测结果优于单一标志物检测。

     

    Abstract: Objective: To detect the micrometastases in peripheral blood of patients with breast cancer using MUC1 and CK7 as a gene marker and to discuss the clinical value. Methods: MUC1 andCK7 mRNA expression of the nucleated cells in circulating peripheral blood were analyzed using re-verse transcriptase polymererase chain reaction (RT- PCR). Results: a) 15 normal peripheral blood samples were detected and, it turned out that no MUC1 and CK7 mRNA expression was false- positive;breast cancer cells MCF-7 were mixed with normal peripheral blood nucleated cells at the concentra-tions of 1:10 5, 1:10 6, and no MUC1 and CK7 mRNA expression was detected to be false-negative. b) Of the 35 patients with breast cancer,11 (30.3%) were positive for MUC1 mRNA expression and 12 (35%)were positive for CK7 mRNA expression of the nucleated cells in circulating peripheral blood. c) Of the 35 patiens, 4 were positive for both MUC1 and CK7 expression, 7 were positive for only MUC1, 8 were positive for only CK7 and the patients positive for both two markers were crossed but not overlaped.Conclusion: Using RT-PCR to detect micrometastases in peripheral blood of the patients with breast cancer, with MUC1 and CK7 as a gene markers, can be used as a prognostic indicator and the multi-marker detection were better compared to single marker.
