HIF-1α P-gp在腋淋巴结阴性乳腺癌组织中的表达及意义

  • 摘要: 目的:检测缺氧诱导因子-1α(HIF-1α)、P-糖蛋白在腋淋巴结阴性乳腺癌组织中的表达情况及其与乳腺癌分型、分级及预后的关系。方法:采用免疫组织化学方法检测81例乳腺癌和20例癌旁组织中HIF-1α、P-gp的表达状况,分析与各临床病理因素和预后的关系。结果:HIF-1α在乳腺癌中总阳性表达率为69.1%,其中强表达10例(12.3%),中度表达12例(14.8%),弱表达34例(42.0%),隐性25例(30.9%);81例乳腺癌组织中34例P-gp表达阳性,占42.0%,其中强表达7例(8.6%),中度表达9例(11.1%),弱表达18例(22.2%),阴性47例(58.0%)。HIF-1α和P-gp在乳腺癌中两者阳性指数呈正相关(r=0.62,P<0.01)。HIF-1α为单独影响乳腺癌预后的危险因素。结论:HIF-1α和P-gp两者阳性指数在乳腺癌中呈正相关;HIF-1α可作为判断乳腺癌恶性程度、预后的单独指标,高表达者5年生存率低,检测HIF-1α有助于提高评估腋淋巴结阴性乳腺癌术后生存的准确性。

     

    Abstract: To study the expression of HIF- 1αand P- gp in breast cancer and to determine the correlation with prognosis. Methods: Formalin - fixed, paraffin - embedded samples from 81 breast cancer patients were tested for HIF- 1αand P- gp expression by immunocytochemical staining. Then the influence of these expression levels on the clinical and pathological parameters and on survival was investigated. Results: Strong expression of HIF- 1αwas observed in 10 cases (12.3%), while expression was moderate in 12 cases (14.8%) and weak in 34 cases (42.0%). No expression was found in 25 cases (30.9%). Strong expression of P- gp was observed in 7 cases (8.64%), moderate expression was observed in 9 cases (11.1%) and weak expression was observed in 18 cases (22.2%). No expression was found in 47 cases (58.0%). HIF- 1αexpression had a significant positive correlation with the expression of P- gp (r=0.62, P<0.01). In the 64 cases with follow- up data, univariate analysis revealed that the survival rate was correlated with histological grading, staging, tumor size, and expression of HIF- 1αand P- gp. However, Cox stepwise proportional hazards analysis showed that only HIF- 1αwas considered independently significant for predicting patient outcome. Conclusions: The expression of Pgp may be correlated with HIF- 1αexpression. HIF- 1αexpression is correlated with the prognosis of breast carcinoma. The expression of HIF- 1αcan be used to accurately assess the prognosis in breast carcinoma and may be helpful for evaluating the prognosis of ANNBC.
