甲状腺髓样癌外科处理与基因筛查原则探讨

Surgical Manipulation of Primary Foci of Medullary Thyroid Carcinoma

  • 摘要: 目的:分析不同类型甲状腺髓样癌手术治疗结果,探讨甲状腺髓样癌原发灶个体化治疗的最佳术式及基因筛查作用。方法:回顾分析我院1992年1月-2005年1月收治的91例甲状腺髓样癌患者手术治疗结果,统计不同类型患者甲状腺叶发病情况,对部分遗传型甲状腺髓样癌家系进行基因筛查。结果:91例甲状腺髓样癌患者中遗传型10例,其中6例初始病变为双侧,4例初始病变为单侧患者手术治疗后随访已有3例对侧腺叶发现癌瘤,遗传型甲状腺髓样癌双侧腺叶受累比例高达90%。散发型甲状腺髓样癌患者81例,其中初始病变累及双侧腺叶者11例,余70例初始病变为单侧的散发型甲状腺髓样癌患者,术后随访4例患者对侧腺叶发现癌瘤,散发型甲状腺髓样癌患者双侧腺叶受累比例为18.5%。行基因筛查检测出RET突变基因携带者3人。结论:遗传型甲状腺髓样癌家系成员应行基因筛查,遗传型甲状腺髓样癌患者和双侧发病的散发型甲状腺髓样癌患者应行全甲状腺切除术,对无明确家族史、术前影像学检查考虑单侧病变的患者可行单侧腺叶加峡叶切除术。

     

    Abstract: Objective: To analyze the results of surgery on different types of medullary thyroid carcinoma (MTC) and to explore the optimal surgical manipulation of primary foci. Methods: We retrospectively reviewed data from 91 patients with MTC who were treated with surgery in Tianjin Cancer Institute and Hospital from 1992 to 2005. Germline RET proto-oncogene mutation analysis was performed in family members of patients with hereditary MTC. Results: Of the 91 cases of MTC, there were 10 cases of hereditary MTC (6 cases with bilateral tumors and 4 cases with unilateral tumors). Of the 4 cases with unilateral tumors, 3 cases had tumor recurrence in the contralateral gland during follow-up. Bilateral MTC occurred in 90.0% of patients with hereditary MTC. Bilateral tumors were found in 11 of 81 sporadic MTC patients. The other 70 patients with sporadic MTC had unilateral tumors. During follow-up, 4 in 70 cases had tumor recurrence in the contralateral gland. Bilateral MTC occurred in 18.5% of patients with sporadic MTC. Of those relatives of patients with hereditary MTC who were screened, 3 were identified with the RET proto-oncogene mutation. Conclusion: Family members of patients with hereditary MTC should be genetically screened. Total thyroidectomy should be performed in hereditary MTC patients and in sporadic MTC patients with bilateral tumors. For sporadic MTC without evidence of bilateral tumors, lobectomy with isthmectomy is the appropriate treatment.
