肺腺癌和鳞癌FDG摄取与临床分期关系研究

The Relationship between Fluorodeoxyglucose Uptake and Clinical Stage in Patients with Adenocarcinoma or Squamous Cell Carcinoma in the Lung

  • 摘要: 目的: 对比肺腺癌和鳞癌FDG-PET/CT显像,研究肺腺、鳞癌的原发灶FDG摄取与肿块大小、临床分期的相关性。 方法: 2004年6月~2007年4月治疗前行FDG-PET/CT检查的拟诊肺癌患者中,并经病理或细胞学诊断证实的腺癌患者94例和鳞癌患者65例,比较二者原发灶大小(最大径表示)和FDG摄取(SUVmax)的差异,再进一步按照不同分期分组,分析腺、鳞癌原发灶FDG摄取与肿瘤大小以及临床分期的相关性。 结果: 鳞癌原发灶的最大径大于腺癌(3.87±1.83cmVs3.08±1.56cm,P=0.004);鳞癌原发灶的SUVmax也大于腺癌(11.32±5.59Vs9.22±4.73,P=0.012),鳞癌和腺癌的大小都与SUVmax呈显著正相关(分别r=0.587,0.652;P均=0.000),腺癌不同临床分期间的SUVmax有显著性差异(P=0.000),而鳞癌不同临床分期间的SUVmax无显著性差异(P=0.662),进一步相关性分析表明腺癌的SUVmax与临床分期呈正相关(r=0.420,P=0.000),鳞癌SUVmax与临床分期无相关性(r=0.083,P=0.509)。 结论: 肺癌FDG摄取受肿瘤大小和病理类型的影响,肺腺癌的FDG摄取与临床分期正相关而肺鳞癌的FDG摄取与临床分期无明显相关。

     

    Abstract: Objective: To investigate the relationship between fluorodeoxyglucose(FDG) uptake and the clinical stages of adeno-carcinoma(AC) and squamous cell carcinoma(SCC) in the lung. Methods: We reviewed the data from 94 cases of AC and 65 cases of SCC who underwent FDG PET/CT in the nuclear medicine center of our hospital between June 2004 and April2007. The differences in FDG uptake and tumor size between AC and SCC were evaluated. The correlation between SU-Vmax and tumor size or clinical stage was also analyzed. Results: Tumor size and SUVmax were significantly higher in SCC patients than in AC patients. (P=0.004 and P=0.012, respectively). There was a positive correlation between SUVmax and tumor size in both types of patients(P<0.001). Significant differences were found in SUVmax among different stages of AC(P=0.000). A significant correlation was found between tumor stage and FDG uptake value for AC patients (r=0.420, P=0.000). Conclusion: FDG uptake of AC and SCC tumors in the lung are influenced by histologic subtype. SUVmax of AC is correlated with stage.
