前列腺癌并发上尿路积水的诊断和治疗(附32例报告)

The Diagnosis and Treatment of Hydronephrosis of Upper Urinary Tract Caused by Prostate Cancer (32 Case Reports)

  • 摘要: 目的:探讨前列腺癌并发上尿路积水的诊断和治疗方法。方法:回顾性分析32例前列腺癌并发上尿路积水患者诊断、治疗方法及疗效。结果:32例患者中行IVU5例、经直肠B超32例、CT23例、MRI15例。32例均行全雄激素阻断治疗,血液透析4例,上尿路分流术11例。32例患者中28例治疗前血清PSA值平均47.73ng/ml(6.00~120.00),治疗后血清PSA值平均14.17ng/ml(0~38.40),平均下降33.56ng/ml(3.40~111.40)。B超示肾积水改善19例,未有明显变化10例,积水加重3例。经治疗后3个月,除1例死亡外,14例受损肾功能均有不同程度的改善。结论:B超是诊断前列腺癌并发上尿路积水的主要方法。内分泌治疗在前列腺癌并发上尿路积水的临床处理中占有重要的地位。

     

    Abstract: Objective: To suggest a definitive diagnosis and therapy for hydronephrosis of the upper urinary tract caused by prostate cancer. Methods: The diagnosis and therapy data in 32 patients with hydronephrosis were retrospectively analyzed. Results: There were 5 intravenous urographs (IVU), 32 transrectal ultrasounds (TRUS), 23 CT scans, and 15 MRI scans for these patients. All patients had been treated with complete androgen deprivation, and 4 patients had hemodialysis. After 3 months of therapy, serum PSA decreased in 28 patients from a mean of 47.73 ng/ml to 14.17 ng/ml. Hydronephrosis improved in 19 patients, remained unchanged in 10, and deteriorated in 3. Renal failure was reversed in 14 patients. Conclusions: Ultrasound is the main diagnosis method for hydronephrosis caused by prostate cancer. Androgen deprivation plays a very important role in treatment for hydronephrosis of the upper urinary tract in prostate cancer patients.
